What is European Health Care Card?

The European Health Care Card is a card that will allow you to get medical service during your stay in EU.

The card was first issued in 1.06.2004 in some countries instead of E 110, E 111, E 119 i E 128 forms. In other countries, including Poland, the card is issued since 01.01.2006.

Since 1st of January 2006 E111 forms have expired and only the card is valid.

Notice:  The card does not state that You are insured by NFZ (Narodowy Fundusz Zdrowia) and does not allow You to get medical service in Poland. This means that person insured by NFZ can use the card only in other EU countries.

What personal information is included on the card?

Those information are included on a card:

  • name;
  • surname;
  • date of birth;
  • PESEL;
  • ID no. of an institution that issued the card;
  • ID no. of a card;
  • Expire date;

Those are the only informations included on a card. Each EU country issues the card in it's official language, including the same set o information.

 

Prices and fees of medical care:

During the first visit to Your family doctor in a term (quarter) You will be asked to pay 10 Euro, during next visits You will only have to present the payment confirmation.The same goes for dentist and any other medic. A visit to a medic consultant with a relegation from Your family doctor is free otherwise You will have to pay 10 Euro. An additional fee of 10 Euro per day spent in a hospital, no longer than 28 days, will be colected. Moreover You are required to pay a for any above-standard services in the hospital.

Rehabilitation fees are 10 Euro per day.

The price of prescipted (Kassenrezept) medications is 10% of value but not less than 5 Euro and not higher than 10 Euro. If a cheaper version of drug exists You will be asked to cover the difference in price. For drugs on other prescriptions You will have to pay the full price.

Drugs for pregnant women are free.

Medical care for children below 18 years old is free, the same goes for drugs.

 

Medical transportation.

For medical care call - 112. Medical transportation in Germany belongs to companies not connected with hospitals You will be required to show them Your health care card. If You will not have it on You You will have to send a copy of the card to an institution issuing the transportation bill.
For transportation to a hospital in German there is a fee of 10% the value, not less than 5 Euro but not higher than 10 Euro. Medical transportation fees will be refunded only in special causes.

For medical transportation to Polan You will have to pay the full price.

Notice: The card does not cover the medical transportation costs to Poland it is advised to get an additional insurance.
 

Prescriptions:

Only a medic who has signed a contract wit a Kasa Chorych is allowed to give You a prescription.

 

Private medical care:

If You do not show aprpriate paper You will be charged full cost as a private customer. For prescriptios issued this way You will have to pay the full price.
The medic will give You the money back if You bring him the card by the end of the term (quarter). With the dentist the period is 10 days starting from the day of Your visit.

 

Papers needed for the refund:

You can get the refund when You return to Poland. You have to send a proposal to the approporiate for You  province-NFZ. To get a refund You have to deliver the payment confirmations and prescriptions (copies are accepted).
